if someone takes off their tefillin for some reason, like to shower or for a break in the middle of the day, and puts them back on later, do they say the bracha again?
and does it matter how long they were off, or why they came off, or if the person stopped thinking about them in between?

If you take it off for a minute or two in order to scratch your head, don’t make another Berachah. If you take it off to go to the bathroom or in order to do something for several minutes or more, make another Berachah.
